New impulses: "Cross-sector stroke care – where do we stand, where do we want to go?"

Alongside topics such as optimisation of secondary prophylaxis, management of participation restrictions and advisory needs, the substitution of neuropsychological treatment needs by occupational-therapy-activating measures and the timely provision of assistive devices were also discussed.

Prof. Dr. Gudrun Zimmermann, professorial representative of the Occupational Therapy programme at IB Hochschule für Gesundheit und Soziales, gave impulses on the topic "Outpatient neurocognitive therapy: future role of occupational therapy?"

The discussed cross-sector care is intended to ensure the necessary continuity of care in an evidence-based and qualitatively even better way and to reduce repeated stroke events.

SASA (Schlaganfall-Allianz Sachsen-Anhalt / Stroke Alliance Saxony-Anhalt) started in March 2015 as a cooperation project between the University Hospital Halle, the Medical Faculty of Martin Luther University Halle-Wittenberg, and Strehlow GmbH. In the context of healthcare developments, which also include an increase in strokes, new ways are needed in healthcare. Those affected by a stroke still often experience breaks in post-acute care, e.g. through long waiting times for outpatient therapies, therapy interruptions, mis-fitting of assistive devices or missing adequate secondary preventive measures. The Stroke Network Saxony-Anhalt (SASA) addresses this interface problem between inpatient and outpatient care of those affected in the post-acute phase after stroke.
